The herbert screw is actually a series of headless screws which are completely implanted in the bone and, so do not stick out at all whereby they could cause any surrounding tissues irritation. Being cannulated to carefully place these screws in positions that will offer compression maintaining the fracture stable, it is because of this reason that many surgeons prefer them. Cannulation improves the accuracy of screw placement during the procedure.

The herbert screw is very commonly employed for fixation of small bone fractures, osteotomies, non-unions, and arthrodesis. Most commonly, it is used in fractures of the scaphoid, metatarsals, carpal and metacarpal bones and intraarticular fractures of the tarsals.

The technique runs generally as follows: planning and inserting a guide wire, determining screw length, predrilling, countersinking, and finally inserting the screws. This cautious approach makes sure that each screw goes right to give maximum compression and support to the fragments of the bone.
